*Base Wage Starting at: $20.25


Essential Functions:

• Assist with referral tracking and feedback
• Assist with performance improvement and data management
• Process enrollment, discharge and transfer forms
• Assist with enabling services such as transportation, food, and housing
• Assisting patients with appointment reminders, filling of prescriptions, follow up on self-management and goals
• Other duties as assigned

• Ability to communicate clearly and effectively with all levels
• Ability to complete competency for CPR/First Aid
• Ability to prioritize and complete multiple tasks simultaneously
• Very effective oral and written communication skills; customer service

Working Conditions:

• Exposure to distressed patients, families or visitors
• Possible exposure to inside environmental conditions

Physical Demands:

• Requires standing, sitting or walking for up to 8 hours or longer per day
• The ability to lift up to 50 lbs

Qualifications:

• High school graduate or equivalent

Preferred Qualifications:

• Experience with community outreach and marketing
• 4 year degree in Human Services
