Based on Experience:
*Range for 2 year degree $27.29-$43.40
*Range for 4 year degree $28.30-$44.97

Essential Functions:

• Builds relationships with patients and coordination with primary care team, specialty care teams, and inpatient facilities. 
• Develops care plans for patients with Program. 
• Monitors HIT tools and reports to identify gaps in care and needed services for patients within program. 
• Addresses medication alerts, hospital admissions and discharges, including emergency visits. 
• Monitor and report performance measures and outcomes. 

Skills Abilities:

• Ability to communicate clearly and effectively with co-workers and patients
• Ability to complete competency CPR and First Aid
• Ability to prioritize and compete multiple tasks simultaneously.
• Effective oral and written communication skills.

Working Conditions:

• Possible exposure to inside environmental conditions.
• Exposure to distressed patients, families, or visitors.

Physical Demands:

• Requires standing, sitting or walking for up to 8 hours or longer per day.
• The ability to lift up to 50 lbs.

Qualifications:

• Active Missouri RN license 

Preferred Qualifications:
 

• Clinical experience preferred but not required
• PCHH and/or Chronic Care Management nurse experience
