Summary of Duties:

Assist in the daily needs of patients including the participation in interdisciplinary team treatment plans on the Behavioral Health Unit (BHU). Provide observation, ongoing assessment and therapeutic intervention consistent with physical and psychological status. Provide individual and group therapy for patients in order to achieve therapeutic goals. Act as a back up to Case Management while also maintaining patient records in the Electric Medical Record (EMR) of all interventions, therapies, referrals and other resources/supports provided during patient treatment. Participate in the treatment/care plan, initial assessment, level of care considerations and discharge planning including planning treatment goals with patients & their families.

Essential Functions:

• Participates in the social service needs with patients on a daily basis. Identifies needs for the patients and assist them in communication by providing quality counseling (group & individual) to patients while assisting them in adjusting to life through individual and group sessions. Maintaining reports of patients’ medical histories, and monitoring changes in their condition.
• Assist in the delivery of care to patients by monitoring practices, educating, providing appropriate resources and providing care in a cost effective manner. Focus on enhancing the quality of care provided, improving outcomes, as well as improving patient, family and staff satisfaction.
• Attends staff meetings as required.
• Provides coverage for Case Managers in their absence, which will include coverage for the hospital Social Workers if needed.
• Participates in Treatment Planning process from admission to discharge meeting the (3) three day requirement. Completes Biopsychosocial assessments within the time frame identified per regulations.
• Attends treatment team meetings regularly and provides meaningful input.
• Participates in the intake /admission process and will follow protocols for admission/denials understanding EMTALA.
• Evaluate and make records of patients’ progress in line with standard goals illustrated in care and treatment plan.
• Address patient concerns and goals while maintaining communication with the patient as well as a case manager.
• Acceptance of additional duties as assigned.

• This individual shall be able to work well in a structured team environment with a variety of disciplines.
• This individual shall be able to utilize best practice procedures to assist in the care and wellbeing of patients served and address clinical and ethical dilemmas in a professional and competent manner.

Physical Demands:

• This individual will have the ability to occasionally lift up to 50 pounds.
• This individual will have the ability to stoop, kneel, crouch, reach, stand, walk, push, pull, and grasp objects as needed.
• This individual will have the ability to perform repetitive motions with wrists, hands, and fingers

Qualifications:

• MSW or other relatable degree in counseling, public health, or human services field with appropriate license ( e.g., LMSW, LCSW, or LPC)
